How we CARE for our Caregivers.

Coaching

We'll be there to help answer your questions about in-home family caregiving and provide much needed support through nurse-led care plans. 

​

Assistance 

Receive financial assistance in the form of a tax-free stipend paid to you weekly. Each SFC agency determines how much they'll pay their Caregivers, typically around 50% of compensation received from Medicaid Waiver. We pay our Caregivers approximately 60%. CARE Team believes Caregivers should be paid more!  

Level 1=$46.50/day

Level 2=$60.00/day

Level 3=$80.00/day

*Level of service is determined by your local Area Agency on Aging (AAA) Case Manager.

We connect you with community resources in your area based on your specific needs.

Our CARE Coaches will ensure you have the information you need to provide the best care to your loved one.

Client

  • Have medical needs requiring nursing facility level of care

  • Live in the same home as the caregiver

  • Be enrolled in Medicaid A&D Waiver *effective July 1st, A&D Waiver will transition to the Health & Wellness Waiver and for age 60 yrs and over Pathways for Aging

Caregiver

  • Live in the same home as the client

  • Are at least 18 years of age

  • Able to pass a criminal background check

  • Have or obtain a TB test

  • Be willing and able to privide care
